Bring all of your pills , both prescription and over the counter medications.
Bring a list of any health professionals you have seen since your last visit to us.
Bring a companion if you have trouble hearing, remembering or understanding instructions given to you by your doctor.
Bring a list of questions for your doctor- we may not be able to address all of them at the time of your appointment but we will make arrangements to address them as necessary.
Bring a wheelchair or walker to help you into the building if necessary- the office is fully wheelchair accessible.
Wear clothing that is suitable for ease of examination.
Avoid wearing perfumes , cologne or scented products as some people are quite sensitive to fragrance.
Please feel free to use the hand sanitizer and masks in the entrance to our offices if appropriate to your condition.
Make any necessary followup appointments prior to departure from our office and store the appointment card in a readily available place.
Notify as soon as possible if unable to keep your appointment or are going to be late.
