Don’t have a family physician but need one?
Dr. Francis LeBrun is NOW accepting new patients who are WITHOUT a family physician. You must live within a 35KM radius from our clinic at 512A Main St, Winchester, ON. You will also need a valid OHIP card and version code in order to apply. An invalid OHIP card will delay. your application.
Step 1:

After you have completed the “My Demographics” information please DO NOT CALL our office for an update. We will be reaching out to let you know what the status of your application is. Continue to use your current health care method until we notify you that you have been accepted as Dr. LeBrun’s patient.
STEP 2:
Please wait for our office to connect with you. If accepted, we will be sending you (5) forms to complete.
a) Email consent form
b) New patient health history intake form
c) “Letter to Release Personal Health Info”.
d) Nation River Health Clinic practice policy.
e) Nation River Health Clinic privacy policy.
We also require a current list of medications from your pharmacy in order to proceed. Feel free to upload your list to us from the button below or have the pharmacy fax us at (613) 774-0916.
STEP 3:
After you have completed ALL of the forms in Step 2 we will reach out to have you sign Ministry of Health “Roster” forms to make your relationship with Dr. LeBrun official.
We will also be letting you know if we require you to come in for a “Meet & Greet” appointment with Dr. LeBrun to review your health history or if you can start seeing the doctor as need be.
